The management of anterior cruciate ligament (ACL) injuries saw a transformative shift in June 2022, with a strong emphasis placed on pre-rehabilitation and mental health as critical components of recovery. New consensus guidelines introduced in January had already begun reshaping clinical approaches, but by mid-year, the practical application of these recommendations was gaining momentum across hospitals and athletic organizations.

One of the most significant advancements highlighted was the implementation of pre-rehabilitation (pre-rehab) protocols. These involve targeted physical therapy to enhance strength, improve joint range of motion, and condition the affected area even before surgical intervention. Clinical studies have shown that patients who engage in structured pre-rehab programs tend to experience better postoperative outcomes, including faster recovery times, improved mobility, and lower risk of complications.

In tandem with physical preparation, the psychological readiness of patients undergoing ACL surgery emerged as a vital factor in preventing re-injury. Surgeons and sports psychologists noted that individuals who enter recovery with high levels of anxiety, depression, or lack of confidence are significantly more prone to setbacks. To address this, hospitals began introducing comprehensive care models that include mental health support alongside physical therapy. These integrated clinics offer services such as counseling, stress management, and mental skills training to help patients maintain a positive mindset throughout their recovery journey.

Preventive strategies were also expanded, drawing from elite sports practices. Medical staff adopted dynamic warm-up routines inspired by FIFA’s Prevent Injury and Enhance Performance (PEP) program. These routines focus on improving neuromuscular control, balance, and flexibility, which are crucial for minimizing the risk of non-contact ACL injuries. Athletes and coaches were trained in these techniques, promoting injury prevention as a cornerstone of long-term athletic health.

By the middle of June, the adoption of a mandatory “pre-hab plus psychological screening” protocol for all ACL procedures was endorsed by medical directors across several professional sports leagues. This represented a groundbreaking step toward standardizing a holistic approach to ACL injury management. These new pathways ensure that every patient receives a baseline level of care that addresses both the body and the mind, ultimately reducing the recurrence rate and enhancing the quality of life post-recovery.

This integrated model reflects a growing recognition that successful recovery from ACL injuries requires more than just surgical expertise. It calls for a multidisciplinary approach that empowers patients physically and mentally, setting a new benchmark for sports medicine and orthopedic care.
